About Continuum Nutrition Partners

Continuum Nutrition Partners, LLC was created to make specialized nutrition care more accessible to patients receiving care at home and to the healthcare organizations supporting them.

We partner with home health agencies, hospice organizations, and other healthcare organizations to integrate evidence-based nutrition expertise into patient care. Our services include comprehensive nutrition assessments, medical nutrition therapy, parenteral and enteral nutrition support, chronic disease nutrition management, and nutrition care for patients with complex clinical needs.

 

We also work alongside healthcare organizations to strengthen their nutrition programs through staff education, clinical consulting, documentation support, quality improvement initiatives, and the development of practical tools and processes that help teams identify and address nutrition risk earlier.

At Continuum Nutrition Partners, we believe nutrition is an essential part of the patient care continuum. By collaborating with patients, caregivers, nurses, physicians, and interdisciplinary care teams, we help translate nutrition recommendations into practical strategies that support healing, improve quality of life, and help patients remain safely in the place they call home.

Our Story

Continuum Nutrition Partners was founded from a deep understanding of home-based healthcare and two decades of experience caring for patients with complex nutrition needs.

 

We recognized a significant gap: patients receiving care at home often have nutrition concerns that can affect healing, strength, quality of life, and their ability to remain safely at home, yet access to specialized dietitian support can be limited. CNP was created to help close that gap by bringing clinical nutrition expertise directly to patients and the healthcare organizations caring for them.
